It might sound cliche, but that is the fact.
On a daily basis, dedicate specific time to Listen, Read and Write English. Listen to speeches of leaders, politicians, celebrities - not just Indian but include all English speaking countries. Watch movies - I may recommend classics from black and white era (exclude films with slang). Read books.
The activity of reading and writing will help you understand/learn the sentence structure - regarding grammar and vocabulary. Make these activities part of your daily schedule. You can see improvement in your communication.
