When someone carries a heavy box, can he say "it's a real pain" in spoken English?

Yes. That is appropriate usage. Anything that is inconvenient can be "a real pain". For instance, it's a real pain to study for this test OR it's a real pain to get up this early.

Remember that even highly paid celebrities make mistakes. "Wanna, like, gotcha" are not english words. If they can cook up those words and use them with style, there's nothing that you can do wrong either! Cheer up, chin up and speak up, I say!!

Yes that is fine. Remember the difference between "It's" and "Its" while writing it. Another way of expressing it is "Boy, that's heavy!". There are several other figure of speech that one can use. Thanks.

What is the difference between spoken English class and IELTS Class?
Spoken English class is for English speaking. IELTS is for study or working in abroad(English speaking countries) and also speaking is comes under a part of IELTS
