When someone carries a heavy box, can he say "it's a real pain" in spoken English?
Yes. That is appropriate usage. Anything that is inconvenient can be "a real pain". For instance, it's a real pain to study for this test OR it's a real pain to get up this early.

How to i learn spoken english?
English is easy language. To learn any language you need to develop vocabulary. Try to learn the English word of all the things around you. Try to make small sentences. Most importantly - listen. Start with cartoon channels. Best of luck.
