The Pitch Fork is also know as Tuning Fork. The pitch that a particular tuning fork generates depends on the length and mass of the two prongs. It is frequently used as a standard of pitch to tune a guitar. Whereas pitch pipe is a small pipe blown to set the pitch for tuning a guitar.

Pitch fork is a U-shaped metal fork which produces sound by means of mechanical vibrations of the fork when you hit it. A pitch pipe is a wind instrument pipe that produces sound by air compressions when you blow through it
