Start with any referenced book of gre math and practice as much as possible and lastly five year question.......

No there is no such exam but in few cases Subject GRE test is recommended.

Answer to Q.2 It is better to continue MS immediately after your UG. Since you are going to specialize in the area of your interest a break is not advisable. In case you are going to pursue MBA, then experience becomes necessary.
