For being efficient in spoken English, you don't need to speak English all the time. Of course, without practice, we cannot achieve fluency in any language. Lack of training can also decrease your efficiency in a language.
Some method which may help you are trying to think in English, Talk to your self and record it, talk to the mirror, try to make friends with whom the only way of communication is English in any social media platform.
